DATE FULLY FUNDED: 7/1/2016

TREATMENT: HepB, Rotavirus, DTaP, and Pneumococcal conjugate vaccinations were given. We also provided Eshan with an afternoon snack.

OUTCOME: We explained to Eshan's parents the effects of the vaccinations, as well as possible side effects such as swelling or joint pain. We provided our physicians' contact information should they have any concerns.
